This application is currently accessible to residents of various counties in New York, including Albany, Allegany, Broome, Cattaraugus, Cayuga, Chautauqua, Chemung, Chenango, Clinton, Columbia, Cortland, Delaware, Dutchess, Erie, Essex, Franklin, Fulton, Genesee, Greene, Hamilton, Herkimer, Jefferson, Lewis, Livingston, Madison, Monroe, Montgomery, Niagara, Oneida, Onondaga, Ontario, Orleans, Oswego, Otsego, Putnam, Rensselaer, Rockland, Saratoga, Schoharie, Schuyler, Seneca, St. Lawrence, Steuben Suffolk Sullivan Tioga Tompkins Ulster Warren Washington Wayne Westchester Wyoming and Yates. However, it is important to note that this application does not provide monitoring for emergencies. In case of an emergency situation that requires assistance from your Social Services District office directly. You should refrain from using this application to submit initial applications for SNAP, HEAP Temporary Assistance and Medicaid or any periodic reports or recertification applications.

Moreover, sensitive images containing information such as HIV status or domestic violence details and/or confidential addresses should not be submitted through this application. For the protection of you or anyone in your household whose safety requires confidentiality or if the application is inaccessible at the moment you should provide such documents through alternative methods such as U.S. Postal Service or through an in-person visit or a kiosk (if available) or by using a fax machine.
